I did end up buying that cover on Amazon. I got the full part that included the power button with integrated finger print reader and camera lens cover. Don't want to have to transpose anything or otherwise piecemeal this install. $27.81 with tax.
And I just completed the rear cover replacement.
It took a bit of effort to crack the seal and ultimately I did use a utility blade but after that I was able to work around it with plastic stuff while applying heat (aimed for about 200F with temp gun checks) which would have been way easier with another set of hands. But I did get it off. It cracked quite a bit more but so what.
Also notice the date - Sept 2017. So the phone is actually over 3 years old. I got it in Jan 2019 (for $220) so approaching 2 years of ownership. I also removed all the old glue, which was in some weird places. But I simply looked at the old and new covers to figure out where the glue was going to be and removed it. Some areas were not obvious (like not around the perimeter). I also caught that there is a little pad right under the power button. It stayed with the phone so if I put the new cover on, it would have had a double pad and possibly bulged a little. Some Amazon reviews mentioned bulging and most likely they just didn't pay close enough attention to preparing the phone body by removing all the old stuff. Fitment seems perfect. The power button works the same as before, and the finger print reader works the same, too. I cleaned the cameras, and also the back side of the new lens covers.
Oddly this is now a "Verizon" phone but that's not really a problem. I suppose it could make resale a little tricky but I'll cross that bridge when I get to it, and that's if I even decide to sell it. These last pics are actually from my phone. Nice and clear again. It is my hope that the new cover with adhesive with "factory" installed power button and lens cover assembly will finally rid this phone of the chronic lens cover lint issue that has plagued it pretty much since day 1. That would sure be nice. And now that the cover is new it no longer looks all smashed.
I guess the goal is just to run it until it is no longer sufficient (or until something breaks that I can't/won't fix). I was sort of coveting Adam's V40 at lunch but I'll stick with my V30 for now. The camera is really pretty decent when not covered in lint.
