If you bought any Silverstar, XtraVision or Cool Blue headlight or foglight bulbs between September 22, 2005 and July 11, 2014, Sylvania will send you $10 due to "unjust enrichment, misrepresentation, negligent misrepresentation, and breach of express warranty". You only have until November 14, 2014 to submit the form.
