I think most cases are either this, or something jamming the gas down, like a loose floor mat. I don't know that any car has actually had a software error that lead to unintended acceleration, even for Toyota. Hardware, sometimes. Old cars had throttle linkage and with old engine mounts that let the engine twist too much, those were known to jam open. We're talking 1960s here. Also, for Toyota, I think they actually had some sticky throttle pedals (drive by wire).

But as this video shows, I think most people just hit the wrong damn pedal (and will never admit it).

Most of the Toyota recalls for those issues involved trimming or replacing floor mats and gas pedals to eliminate potential areas of interference. Something common: owners stacking other floor mats on top of the factory floor mats.
